What DBT is

A therapy for people whose feelings arrive at full volume.

Some people drink or use to chase something. Others do it to get through a feeling that has become unbearable, and they would take almost anything that made the next hour survivable. DBT was built for the second group.

It was developed by Marsha Linehan for people living with intense emotional swings and thoughts of self-harm, and it was later adapted for substance use, where the same pattern shows up constantly. The name points at the balance the therapy is built on: accepting where you are right now, and working to change it, at the same time.

What makes it different in practice is how concrete it is. DBT teaches specific skills, rehearses them while nothing is wrong, and expects you to use them when something is.

The short version

Four sets of skills, taught in a group, applied one to one, and practiced until they hold under pressure.

If previous treatment made complete sense in the room and fell apart the first bad week, that gap is the thing DBT is designed to close.

The four skills

What DBT actually teaches.

Distress tolerance

Getting through the hour without making it worse. The skills here are for the moment the craving or the panic peaks, when the goal is not to feel better but to not act.

Emotion regulation

Noticing what the feeling actually is, earlier than usual, and doing something with it that is not using. Most of this work is about the hours before the crisis rather than the crisis.

Mindfulness

Paying attention on purpose, which sounds soft and is the part everything else rests on. You cannot use a skill you have not noticed you need.

Interpersonal effectiveness

Asking for something, refusing something, and staying in a relationship while doing either. A lot of relapse starts in a conversation that went badly.

Who it suits

You do not need a diagnosis for this to be the right fit.

What matters is the pattern, not the label. A clinical assessment looks at what has actually been happening and your treatment plan follows from that. DBT is offered where it is clinically indicated.

  • Emotions that arrive at full intensity and take a long time to come down
  • Using to get through a feeling rather than to chase one
  • A pattern of acting on impulse and regretting it afterwards
  • Relationships that swing between very close and completely cut off
  • Self-harm, or thoughts of it, alongside substance use
  • Previous treatment that made sense at the time and did not hold under pressure

How it runs

Individual therapy and a skills group, together.

One without the other is half the model. The group teaches the skill, the individual work applies it to your week, and the practice in between is what turns it into something you can reach for.

One

Individual therapy

One to one sessions with a therapist, working on what is actually happening in your week rather than a syllabus.

Two

Skills group

Where the four modules are taught and practiced. It runs like a class more than a support group, and that is deliberate.

Three

Practice between sessions

Skills are rehearsed when nothing is wrong, so they are available when something is. This is the part that makes the difference.

Four

The rest of your treatment

DBT runs alongside detox, residential or outpatient care. It is one part of a plan, not the whole of it.

What is dialectical behavior therapy?
DBT is a structured talking therapy built around four sets of skills: distress tolerance, emotion regulation, mindfulness and interpersonal effectiveness. It was developed by Marsha Linehan, originally for people experiencing intense emotional swings and thoughts of self-harm, and it was later adapted for substance use. The word dialectical points at the balance it is built on, which is accepting where you are now while also working to change it. In practice it combines individual therapy with a skills group.
How is DBT different from CBT?
They share a foundation, and DBT grew out of cognitive behavioral therapy. The difference is emphasis. CBT looks closely at the thoughts driving a behavior and works on changing them. DBT spends more time on tolerating intense emotion without acting on it, and on accepting difficult feelings rather than arguing with them. For someone whose main problem is that feelings arrive overwhelming and fast, DBT often fits better.
Is DBT used for addiction, or only for mental health?
Both. It was developed for emotional dysregulation and self-harm, and there is now a substantial body of work adapting it for substance use, usually where the two occur together. People who use to manage an emotional state rather than to chase a high tend to be well suited to it.
Do I need a mental health diagnosis to be offered DBT?
No. What matters is whether the pattern fits, not whether a particular label is on your chart. Your assessment looks at what has actually been happening, and DBT is offered where it is clinically indicated. A clinical assessment is what determines your treatment plan and level of care.
What does a DBT skills group involve?
It runs more like a class than a discussion group. A skill is taught, practiced in the session, and then used between sessions, with the group working through the four modules in turn. People are often surprised by how practical it is, and by how little of the time is spent talking about the past.
Can I do DBT while I am in detox or residential treatment?
DBT runs alongside the rest of your care rather than instead of it. Where you begin depends on your clinical assessment. If you are medically unstable, detox comes first, and therapeutic work builds from there as you are able to take part in it.
Is DBT available near me?
